Résumé : The integration of Artificial Intelligence (AI) in Home Health Care (HHC) offers a promising approach to enhance patient care and optimize healthcare resources. This report explores the prediction of care needs and the dynamic optimization of home hospitalization routes using AI technologies. The study employs predictive modeling to assess patient care requirements and utilizes optimization algorithms to improve the efficiency of home healthcare delivery. The findings indicate significant potential for AI to improve patient outcomes and operational efficiencies in HHC, paving the way for future advancements in personalized and efficient home-based medical services.
